Bullying, harassment, and intimidation are serious and will not be tolerated. This is a form to report alleged bullying, harassment, orintimidation that occurred on school property; at a school-sponsored activity or event off school property; on a school bus; or in the current school year.If you are a student victim, the parent/guardian of a student victim, a close adult relative of a student victim, or a school staff member and wish to reportan incident of alleged bullying, harassment or intimidation, complete this form and return it to the principal at the student victim’s school. Contact theschool for additional information or assistance at any time.

Enrolling in School & Enrollment Forms
You will need the following items with you to register your child.
- Birth certificate of student
- Proof of custody when warranted
- Copy of parent/guardian driver license
- Proof of residency – two types of documentation must be provided (utility or other type bill/document with the address of the primary residence shown. A post office box is not acceptable proof of residency.)
- Documented proof of immunization as required by state law -
- Alabama forms IMM-50
- IMM-49-temporary
- IMM-51-medical exemption
- IMM-52-religious exemptions
- You may obtain immunization proof from your child’s doctor or health department.
